Access to medical records

If you already have online access to book appointments and to request repeat medications then you can also request to have access to your medical records which will show you your test results and coded information.

If you would like this access added to your records please download the patient information leaflet.  Once you have read this and you are sure you would like this access then please download and complete the access to medical records application form.  Please print this form and bring it to the surgery (either Shoreham Health Centre or Downsway).  We will need to see Photograhic ID.  Once we have received the form this will need to be reviewed by the GP before allowing access to your medical records.

We have the right to decline access when a clinician has decided the information held may harm the patients mental wellbeing.
